By booking an appointment or using services provided by Form & Function, you agree to the following terms and conditions.


Appointment Scheduling

Appointments can be scheduled through our booking system, or through approved scheduling platforms.

Clients are responsible for arriving on time for their scheduled sessions.


Cancellation and Rescheduling

If you need to cancel or reschedule an appointment, please notify us as soon as possible.

Late cancellations or missed appointments may be subject to cancellation policies communicated during booking.


Session Packages

Some services may be purchased as session packages.

Packages may include a set number of sessions that must be used within the specified timeframe. Remaining sessions will be tracked and communicated through appointment updates.

Unused sessions may expire according to the terms provided at the time of purchase.

SMS Messaging Program

By providing your phone number through our booking, onboarding, or intake forms, you consent to receive SMS messages from Form & Function.
